What is a TENS machine?

A TENS machine is a small device that uses electrical currents to help reduce pain. TENS (transcutaneous electrical nerve stimulation) therapy uses electrical nerve-stimulation pulses to block pain signals that come from the nerves (Watson, 2018). Small pads (electrodes) are placed on the skin around the affected areas to deliver these impulses to the body. Different settings are available on portable TENS machines to assist with various types of pain conditions. It helps with sciatica and nerve pain, it relieves migraines and acute pain, it’s good for wound healing, and you can use a TENS during pregnancy and labour.

How do TENS Machines work?

They are a form of electrical therapy used by healthcare professionals, such as physiotherapists and GPs, as a natural form of pain relief for patients. However, TENS can also be used by individuals living with painful conditions, such as lower back pain, muscle spasms, ovulation pain, during pregnancy or sciatica etc.[1].

This portable device is attached to gel pads which are placed on or around the painful areas of the body. Once the TENS unit has been turned on and adjusted, gentle electrical pulses are delivered through the pads. The electrical current creates a tingling sensation which blocks pain signals travelling from the source of the pain to the brain [2].

Currently, how the TENS devices work affects the body is still being researched. However, one proposed theory is that the electrical current provided from the device dulls the effect of the C fibres. These fibres are found underneath the skin to deliver information about any painful stimulus from the affected region (e.g. muscle spasms, inflammation, etc.) [2]. By cutting off the communication from the C fibres via the TENS Unit’s electrical pulses, the painful sensations are reduced and/or removed.

How to use a TENS machine

Before using a TENS machine, it’s important to determine two pieces of information - 1) whether the device is safe for use and 2) what you are using the machine for (e.g. your condition, location of pain, etc.). You may need the assistance of a healthcare professional to determine these pieces of information.

After opening the device from its package, connect the charged TENS device to the electrode padsand apply to the painful area on the body. Set up the unit by either adjusting or selecting a pre-programmed setting according to the treatment you need. Each device should come with a manual to better explain the settings. Some settings you may consider include [1]:

Intensity: Measured in (milliamps) mA, the intensity will determine the strength of the treatment.
Frequency: Measured in hertz (Hz), the frequency will determine how quick the electrical pulses are being delivered.
Pulse width: Measured in microseconds, the pulse width will determine how long each electrical pulse is.

Once these have been adjusted, you can start the treatment. The treatment duration and sessions per day may vary depending on the chosen setting(s). During the treatment, you will feel electrical pulses on the affected area. Adjust the intensity (mA) of the treatment to as much as you can tolerate without any additional pain or discomfort. Continue until the end of the session, or stop if you feel any unusual symptoms or pain.

Difference between TENS and EMS Machines

Transcutaneous electrical nerve stimulation (TENS) and EMS machines may appear very similar due to their outward appearance. Both are handheld devices that deliver electrical current through the attached gel pads. There are also combination units that have both TENS and EMS settings.

However, TENS and EMS machines are used for different purposes [1]. A TENS machine is primarily used to deliver treatments for helping ease acute (short-term) and chronic (long-term) nerve and muscle pain. These machines send unique electrical signals to the nerves to achieve the desired effect. However, an electrical muscle stimulator (EMS) is mainly used to help activate and jumpstart muscles, prepare the body for a workout, increase strength and help prevent muscle wastage. There are many ways EMS can be used, such as improving athletic performance and assisting with rehabilitation.

Users will also report that the treatments also feel different. While the EMS causes a strong contraction of the muscles, the TENS treatment will lead to a repetitive tingling sensation.

Can a TENs machine help anxiety?

Although the main purpose of TENs machines is to reduce pain, it has also been shown to help with anxiety and psychological well being. Health experts suggest that there is a strong relationship between experiencing pain and anxiety. Researchers have found that living with pain increases the likelihood of developing anxiety disorders by 3-9 times (McCracken et al., 1992).

Scientists suggest that helping alleviate pain and reducing disability with the TENS machine can help improve psychological health (Durmus et al., 2009). Some studies have even shown that electrical stimulation significantly improves anxiety in children about to receive dental treatment (Cebalo et al., 2020; Kasat et al., 2014). These children reported a 21.5% drop in anxiety levels (Cebalo et al., 2020). Also, between 53-78% preferred the TENs unit over local anaesthetic (Kasat et al., 2014). It is suggested that the sensation from the device can also double as an effective distraction from negative emotions, such as anxiety.

While the TENs unit for home use is primarily a pain treatment, it also has a therapeutic effect for alleviating anxiety.

Does a TENs machine relax muscles?

Tight muscles or spasms that develop from overuse or repetitive daily activities can lead to aches, pain and poor performance. The term 'knots' describes small focal areas that seem more sensitive and firmer. TENs machines can help relax these areas through various methods, including (Vance et al., 2014):

Increasing endorphins circulating through the bloodstream. These are hormones that can block the body's natural pain-promoting chemicals.
Blunting the pain signals coming from the injured region to the brain.
Reducing the body's sensitivity to pain and discomfort.

Using a TENs unit for muscle spasms and tightness can be effective. With just a 10 minute application of TENs treatment, muscular pain can decrease by 50% (Graff-Radford et a., 1989). When combined with stretching exercises, users noticed that their muscle flexibility significantly increased (Karasuno et al., 2016). Additionally, these same individuals reported having less sensitive and softer muscles.

Are TENS machine good for back pain?

Yes, there are many benefits of using a TENS Unit for back pain. A review of the research by Macahdo et al. (2009) showed that using a TENs device significantly reduced lower back pain across 4 studies and 178 participants. Additionally, people with low back pain who used a TENS Machine for back pain report that they experience significantly increased back mobility and flexibility afterwards (Durmus et al., 2009). These participants were able to perform movements more easily, such as bending forward, sit-ups and lifting their legs with substantially less pain. When combined with exercises, those with back pain have reported that their psychological and quality of life were also significantly enhanced (Jarzem et al., 2005).

Overall, using a TENS machine can help provide immediate back pain relief and associated benefits (e.g. physical function, mental health, etc.). It should be combined with other strategies, such as regular exercise and attending physiotherapy for optimal outcomes.

Can I use a standard TENS unit for labour?

Our standard TENS and maternity TENS work similarly. Although our standard units are intended for general use, specific settings can be selected to assist with labour. However, maternal TENS are easier to adapt and use for mothers when conceiving.

Using any TENS machines should be avoided for at least the first trimester. However, having a standard TENS unit during labour is safe and has demonstrated little to no side effects to the mother or baby (Downswell et al., 2009; Santana et al., 2016). Research has not indicated the need for specific maternity TENS units to significantly reduce pain and the need for pain medications during labour (Santana et al., 2016).

However, there are several significant and practical differences between the two:

BOOST BUTTON:

When a contraction occurs, you need to quickly change the way the TENS operates and then be able to change it back at the end of the contraction. Unlike normal TENS units, the Maternity TENS has a special ‘boost’ button that quickly adjusts the settings when you need them.

INTENSITY CONTROLS:

Most TENS units have a separate intensity control for each lead. The maternity units have one intensity control for both leads to make things simpler.

LARGE ELECTRODE PADS:

During labour, the pain can be widespread. Large areas may need treatment to get effective pain relief. Suppose you do not have the special larger pads which are supplied with the maternity TENS. In that case, the pain relief provided will not be as effective.

In summary, it is much easier to use Maternity TENS during labour as they are specifically designed to suit the needs of pregnant persons.

How effective are TENS Machines? How long does TENS Pain Relief last?

The analgesic (pain-relieving) effects of TENS will vary depending on the selected settings. In most cases, it takes around 30 minutes or less for people to feel these effects. For some settings, such as the ‘briefly intense’ treatment, benefits can be felt as early as 15 minutes (Watson, 2018). Some people prefer to use their TENS for several hours at a time. Our TENS machines have an automatic timer which helps cease treatment so that you can safely fall asleep.

When used on a high-frequency setting, a ‘carry-over’ effect can be felt in users. This can help with pain relief even after the machine has been taken off. The devices can provide relief anywhere between 30 minutes to 18 hours (Woolf, 1991). According to Watson (2018), this can last up to several hours. For some people, pain intensity does not return to pre-treatment levels even after 24 hours (Cheing et al., 2003).

How long can I use a TENS Machine each day?

TENS machines can be safely used throughout the day for as long as required. Depending on the selected setting, the duration of the treatment may vary. For example, lower frequency treatment to help with long-term pain relief will require at least 30 minutes. On the other hand, briefly intense TENs therapy may need at least 15-30 minutes duration to assist with immediate pain relief (Watson, 2018). Overall, sessions can last between 30-60 minutes per session for 3-4 sessions per day.

How Long will Electrode Pads last? Are they reusable?

The electrodes on TENS units can be identified as sticky pads that attach to the skin. These pads are generally reusable for up to 20-30 times if used correctly as directed in our blog. However, this will depend on several factors, such as:

Duration of the treatment. Longer treatments will wear out the electrodes quicker.
Regularly switching between each pair of electrodes between each day of use. This will provide the first set about 24 hours to completely dry, which will expand the usage time.
Regular cleaning of the electrodes helps prevent debris and the build of dirt.
Whether you have a lot of body hair, we recommend shaving areas that need to be treated or keeping it on bare skin.

Where to put TENs machine pads?

It's important to consider that the TENs machine placement has the potential impact on the effectiveness of your treatment. Most TENs machines have two channels that connect to a pair of sticky machine pads. The device can then deliver the treatment to the affected area after proper TENS machine pad placement onto the skin.

For specific areas of pain, most therapists will use one channel (or one pair of pads) around either side of the pain (Watson, 2018). The painful region should be in between the two pads. However, there are also alternative TENs machine electrode placements that may also be effective. Before use, please advise a healthcare practitioner, such as a physiotherapist or osteopath, about these pads' proper usage. These alternative placements include (Neto et al., 2017):

On the opposite side of your pain (e.g. left knee for right knee pain)
Over the areas of the spine where the nerves connect to the painful region
Neighbouring regions where the nerves are related the sore spot (i.e. myotome and dermatome patterns)
Related acupressure points

For more widespread or vague areas of pain, two channels can be used. Place each pair of your pads diagonally across each around the treatment area. Additionally, a combination of alternative TENs machine placements can be used (Watson, 2018). However, please advise a qualified health professional before using.

Can TENS machine cause heart attack?

TENS machines are generally safe for most people. People with cardiovascular abnormalities, heart conditions (e.g. atrial fibrillation, heart disease, etc.) or electrical implants (e.g. pacemakers, cardioverter defibrillator, etc.) should avoid TENS treatment unless informed by their doctor (Rennie, 2010). Additionally, avoid placing the device over the chest due to the proximity to the heart.

Will using a TENS hurt or give me more pain?

If used properly, a TENS will speed up wound healing and not hurt or cause additional pain. Increased pain is usually not a reported side effect unless used over damaged skin (which is not advised to begin off with) (Rennie, 2010; Johnson, 2007)

What are the adverse side effects of TENS machine?

TENS offers a safe and healthy alternative for pain treatment. When used sensibly, serious side effects from TENS machine usage are rare (Johnson, 2010). Being non-invasive and having minimal adverse reactions are unique aspects of pain treatment with a TENS unit.

TENS provides an alternative to traditional pharmaceutical treatments without the potential for dependency. However, some people may have allergic reactions to the electrode pads, such as skin irritation and contact dermatitis (Johnson, 2010). Additionally, the high electrical current can cause further irritation and burns. In this case, special rubber pads are available. In rare situations, nausea and the feeling of fainting may occur (Johnson, 2010).

Always use TENS in a safe and supervised environment, especially during your first use.

A full refund can be requested for defective products wihtin 30 days of the purchase date. You can only choose between an exchange or credit where goods become faulty after 30 days.

Return Postage –

• Defective or faulty items (Dead on Arrival) within 30 days of customer receiving products, the return label will be provided.

• All warranty replacement return and resend postage after 30 days will be at buyer’s expense with registered post.

Shipping is free for online orders over $50 (after any discounts have been applied) or orders under $50 will incur a small delivery fee.

International

How long will it take for my order to arrive?

Most orders are dispatched the same day from Melbourne Warehouse if your order is placed before 2pm, (Monday-Friday) payment is received and products are in stock. Depending on your shipping destination orders can take from 1- 10 business days transit time. Please refer to the following guidelines for shipping timeframes:

Can I collect my online order from the local store?

Yes. You will receive another email notification when your order is ready for collection. To avoid disappointment, do not come to store to collect your item UNTIL you receive a confirmation email. If you come in to collect your order before you receive the email, your order won’t be ready.
